Unique PD shared care model gains momentum
NKF’s PD (peritoneal dialysis) shared care model offered at the PD clinic – which it piloted more than a year ago in partnership with Khoo Teck Puat Hospital and Yishun Community Hospital – continues to gain momentum to better support patients who choose PD as their preferred modality of treatment.
Through this unique model, PD services provided include post-PD catheter insertion flushing, PD training, initiation and maintenance home visits, investigation to check on dialysis adequacy and peritoneum membrane characteristic, medication administration and exit site care. Both hospital and NKF-based nephrologists, nurses, medical social workers, dietitians and allied healthcare professionals ensure patients’ holistic care. Other services including troubleshooting of PD catheter flow issues, PD treatment, home support, clinic review, counselling, respite care and visits by PD nurses are made readily available throughout a patient’s treatment journey. It also allows for more seamless flow of medical information through an integrated electronic system, and shared responsibility for quality control.
The PD clinic can serve up to 30 PD patients a month. More than 100 patients – through a total of around 400 visits – have benefitted from the clinic’s services since it started operations in December 2020.
