Data Protection Notice

The National Kidney Foundation (“NKF”, “we”, “us” or “our”) is committed to ensuring that the personal data collected is managed in accordance with Singapore’s regulations and good practices.

This Data Protection Notice (“Notice”) sets out the basis which NKF may collect, use, disclose or otherwise process personal data of our prospective patients, patients, patients’ authorised representative, donors, volunteers, employees or otherwise in accordance with the Personal Data Protection Act 2012 (“PDPA”). This Notice applies to personal data in our possession or under our control, including personal data in the possession of organisations which we have engaged to collect, use, disclose or process personal data for our purposes.

Collection, use, disclosure or otherwise processing of personal data of our patients on behalf of government ministries or statutory boards will be in accordance with guidelines set out in the government’s data management policy. NKF will comply with the relevant requirements under the government’s data management policy.

Personal Data

  1. As used in this Notice: “personal data” means data, whether true or not, about an individual (whether a prospective patients, patients, patients’ authorised representatives, donors, volunteers, employees or otherwise) who can be identified: (a) from that data; or (b) from that data and other information to which we have or are likely to have access.

  2. Depending on the nature of your interaction with us, some examples of personal data which we may collect from you include name, identification numbers such as NRIC, FIN, work permit and birth certificate, contact information such as residential address, email address or telephone number, nationality, gender, date of birth, marital status, medical history, photographs and other audio-visual information, employment information and financial information such as credit card numbers, debit card numbers or bank account information.

  3. Other terms used in this Notice shall have the meanings given to them in the PDPA (where the context so permits).

COLLECTION, USE AND DISCLOSURE OF PERSONAL DATA

4. We generally do not collect your personal data unless

  1. it is provided to us voluntarily by you directly or via a third party who has been duly authorised by you to disclose your personal data to us (your “authorised representative”) after
    1. you (or your authorised representative) have been notified of the purposes for which the data is collected, and
    2. you (or your authorised representative) have provided written consent to the collection and usage of your personal data for those purposes, or
  2. collection and use of personal data without consent is permitted or required by the PDPA or other laws. We shall seek your consent before collecting any additional personal data and before using your personal data for a purpose which has not been notified to you (except where permitted or authorised by law).

We may collect and use your personal data for any or all of the following purposes to establish or manage your relationship with us. This includes:

    1. Where you are a prospective patient applying into NKF
      • processing and evaluating your application for our services.
    2. Where you are a patient,
      • providing the services requested by you;
      • seeking NKF’s medical services; and
      • monitoring, evaluating and/or auditing of services provided for the purpose of service quality improvement. This may include an assessment of the quality of the services provided and the effects of the services provided (both in the short and long term, and after you have stopped using the services);
    3. Where you are a patient’s authorised representative
      • processing and evaluating the patient’s application for our services.
    4. Where you are a donor,
      • applying to be a donor of NKF;
      • processing your donations and your tax-deduction claims.
    5. Where you are a volunteer,
      • processing and evaluating your suitability to volunteer; and
      • monitoring, evaluating and/or auditing of services you participated in.
    6. Where you are an employee or prospective employee of NKF,
        • administering the processes relating to your employment.
        • Creating content for the purpose of NKF’s outreach
    7. Where you are a member of public,
      • Verifying your identity;
      • processing and responding to your queries, feedback, and complaints;
      • administering events and health screening services; and
      • informing you of upcoming activities and administering the activities;

5. We may disclose your personal data:

  1. where such disclosure is required for, or in connection with, the provision of the services requested by you;
  2. to third party service providers, agents and other organisations we have engaged to perform any of the purposes listed in clause 5 above for us which may include but not be limited to thirty-party doctors, clinics, nursing homes, hospitals, other medical institutions, relevant governmental and/or regulatory authorities.
  3. to comply with any applicable laws, regulations, codes of practice, guidelines, rules or requests by public agencies, or to assist in law enforcement and investigations; and
  4. any other party to whom you authorised us to disclose your personal data to, or where necessary to undertake any action requested by you.

6. The purposes listed in the above clauses may continue to apply even in situations where your relationship with us) has been terminated or altered in any way, for a reasonable period thereafter (including, where applicable, a period to enable us to enforce our rights under a contract with you).

7. We shall seek your deemed consent through SMS, phone call, mail and/or email before collecting any additional personal data or using existing personal data for a purpose which has not been notified to you through a 7 days opt-out exercise. After the lapse of this opt-out period, you may still withdraw your consent by notifying us.

RELIANCE ON THE LEGITIMATE INTERESTS EXCEPTION

8. In compliance with the PDPA, we may collect, use or disclose your personal data without your consent for the legitimate interests of NKF or another person. In relying on the legitimate interest exception of the PDPA, NKF will assess the likely adverse effects on the individual and determine that the legitimate interests outweigh any adverse effect.

9. In line with the legitimate interests’ exception, we will collect, use or disclose your personal data for the following purposes:
a. Fraud detection and prevention;
b. Detection and prevention of misuse of services; and
c. Collection and use of personal data on company-issued devices to prevent data loss.

The purposes listed in the above clause may continue to apply even in situations where your relationship with us (for example, pursuant to a contract) has been terminated or altered in any way, for a reasonable period thereafter.

WITHDRAWING YOUR CONSENT

10. The consent that you provide for the collection, use and disclosure of your personal data will remain valid until such time it is being withdrawn by you or your authorised representative in writing. You or your authorised representative may withdraw consent and request us to stop collecting, using and/or disclosing your personal data for any or all of the purposes listed above by submitting your request via email to the department which you have provided your consent or otherwise in writing to our Data Protection Officer at the contact details provided below. If you are unable to submit your request in writing or if you require any assistance with the submission of your request, you can ask to speak to or meet with our Data Protection Officer.

11. Upon receipt of your written request to withdraw your consent, we may require reasonable time (depending on the complexity of the request and its impact on our relationship with you) for your request to be processed and for us to notify you of the consequences of us acceding to the same, including any legal consequences which may affect your rights and liabilities to us. In general, we shall seek to process your request within seven (7) business days of receiving it.

12. Whilst we respect your decision to withdraw your consent, please note that depending on the nature and scope of your request, we may not be in a position to continue providing our goods or services to you and we shall, in such circumstances, notify you before completing the processing of your request. Should you decide to cancel your withdrawal of consent, please inform us in the manner described in clause 11 above.

13. Please note that withdrawing consent does not affect our right to continue to collect, use and disclose personal data where such collection, use and disclose without consent is permitted or required under applicable laws.

ACCESS TO AND CORRECTION OF PERSONAL DATA

14. If you wish to make (a) an access request for access to a copy of the personal data which we hold about you or information about the ways in which we use or disclose your personal data, or (b) an amendment request to correct or update any of your personal data which we hold about you, you may submit your request via email to the department which you have provided your personal data or otherwise in writing, to the Data Protection Officer at the contact details provided below. If you require assistance with the submission of your request, you can ask to speak to or meet with our Data Protection Officer.

15. Please note that a reasonable fee may be charged for an access request. If so, we will inform you of the fee before processing your request.

16. We will respond to your request as soon as reasonably possible. In general, our response will be within seven (7) business days. If we are unable to provide you with any personal data or to make a correction requested by you, we shall generally inform you of the reasons why we are unable to do so (except where we are not required to do so under the PDPA).

PROTECTION OF PERSONAL DATA

17. To safeguard your personal data from unauthorised access, collection, use, disclosure, copying, modification, disposal or similar risks, we have introduced administrative, physical and technical measures such as:

    1. minimised collection of personal data
    2. authentication and access controls
    3. encryption of data
    4. data anonymisation
    5. up-to-date antivirus protection
    6. regular patching of operating system and other software
    7. securely erase storage media in devices before disposal
    8. web security measures against risks
    9. usage of 2 factor authentication (2fa)
    10. security review and testing performed regularly, and
    11. other measures when necessary.

18. You should be aware, however, that no method of physical and/or electronic transfer or method of physical and/or electronic storage is completely secure. While security cannot be guaranteed, we strive to protect the security of your information and are constantly reviewing and enhancing our information security measures.

ACCURACY OF PERSONAL DATA

19. We generally rely on personal data provided by you (or your authorised representative). To ensure that your personal data is current, complete, and accurate, please update us if there are changes to your personal data by informing the department which you have provided your personal data, or our Data Protection Officer at the contact details provided below.

RETENTION OF PERSONAL DATA

20. We may retain your personal data for as long as it is necessary to fulfil the purpose for which it was collected, or as required or permitted by applicable laws.

21. We will cease to retain your personal data or remove the means by which the data can be associated with you, as soon as it is reasonable to assume that such retention no longer serves the purpose for which the personal data was collected and is no longer necessary for legal or business purposes.

TRANSFERS OF PERSONAL DATA OUTSIDE OF SINGAPORE

22. We generally do not transfer your personal data to countries outside of Singapore. However, if we do so we will take steps to ensure that your personal data continues to receive a standard of protection that is at least comparable to that provided under the PDPA.

EFFECT OF NOTICE AND CHANGES TO NOTICE

24. This Notice applies in conjunction with any other notices, contractual clauses and consent clauses that apply in relation to the collection, use and disclosure of your personal data by us.

25. We may revise this Notice from time to time without any prior notice. You may determine if any such revision has taken place by referring to the date on which this Notice was last updated. Your continued use of our services constitutes your acknowledgement and acceptance of such changes.

General Enquiries

We are open on most public holidays except for the 1st day of Chinese New Year.

NKF does not provide ad hoc dialysis treatment. Our patients are on long-term dialysis treatment. You may wish to check with private dialysis centres for ad hoc dialysis treatments.

NKF does not have an outpatient clinic and hence, we are unable to arrange an appointment for members of the public. You may wish to contact a hospital of your choice for outpatient service.

Currently, NKF does not provide health report interpretation service. You may consult your General Practitioner who will be able to assist you.

Someone with kidney failure may not experience any signs and symptoms until it is too late. It is best to do a routine health screening to understand your health status better and detect any abnormalities as early as possible so that you can delay and manage the disease progression.

All dialysis centres operate on 3 dialysis shifts –morning, noon and evening.

Admissions

Our Admissions Counsellor will contact you once your application is finalised. They will be in contact with you soon.

From the day we receive the application form, it will take approximately 3 working days to a week for us to get back to you if you have submitted all completed supporting documents together with your application form to us. Generally, the whole process will take 6 months for the approval.

The overall application review process will take approximately 6 months before the applicant can start the first dialysis treatment. However, if the applicant is able to provide all completed supporting documents and if there is a slot available at the dialysis centre with a right match to the time slot, the waiting time could be shorter.

Currently, we do not have status checking service online. NKF Admissions Counsellor will be in contact with the applicants regarding the status.

Means Test is used to determine the amount of subsidies each person needing care is eligible for. Persons from lower income households will be granted higher subsidies under the means test framework.

Means Test subsidies calculation is based in accordance to MOH guidelines.

Our NKF Admissions Counsellor in-charge will contact you in regards to your means test status.

The Medical Social Workers at your hospital will be able to advise you on the different type of subsidies based on your financial situation.

Kidney Live Donor Support Fund

For information on kidney donation in Singapore, you may wish to contact the National Organ Transplant Unit at Tel: 6321 4390 or email organ.transplant@notu.com.sg.

NKF’s Kidney Live Donor Support Fund covers annual health screenings and medical follow-ups; one-time reimbursement for loss of 2 months actual income of $8,500, whichever is lower: reimbursement of hospitalisation and surgical insurance premiums (capped at MediShield Life premiums); pre-transplant screening and evaluation costs for up to 2 potential donors per kidney recipient; and insurance coverage for Group Living. Policy based on sum insured of $200,000.

All outright cash donations to NKF are entitled to tax deduction because NKF is a registered charity with an IPC (Institutions of Public Character) status. We are able to issue tax deductible receipts for qualifying donations to our donors.

You are entitled to a tax deduction of 2.5 times the value of your donation in the preceding year.

Once we receive your application, our colleague from the Donor and Volunteer Management Department will contact you for a phone interview. After which, you will be invited to attend our volunteer orientation. This is to allow you to have a better understanding of the volunteering activities.

The minimum age is 13 years old. For volunteers who are below the age of 21, you will need to get parental consent. You may download the consent form here.

We strongly encourage our volunteers to commit at least a year so that we can better support our patients. Our programmes and activities are flexible, you can choose your volunteering schedule according to your interest and availability.

You may volunteer with us basing on your availability. We will send a weekly email to our volunteers indicating the up-coming volunteering opportunities. You can then match your availability to the activity schedule.

Anyone and everyone can join us as a volunteer. All applicants will be required to undergo a mandatory orientation session where you will gain insights into the Foundation’s cause, kidney disease as well as learn more about the volunteering activities we offer. Regular trainings are also conducted for all volunteers so that they can better perform their role.
