Pre-dialysis Education

The Know Right, Start Right programme aims to build active collaboration with restructured hospitals, social agencies and community partners to help Chronic Kidney Disease (CKD) 3 to 5 patients and their caregivers to make informed decisions of their preferred modality and confidently start right on their treatment.

The aim of the programme is to:

  • Ease patients into their treatment regime and provide continuous support
  • Create awareness to patients’ family members and caregivers
  • Reduce the number of patients with unplanned dialysis

Model of Care

Pre-dialysis talks are conducted bi-monthly at the hospital while home visits, peer support and targeted counselling are ad-hoc upon referral by the hospital.
